2017 Identification Number Verification Questionnaire Cycle

Report Deadline Passed - Report Open for Inactivated EPA ID Users

The 2017 electronic Verification Questionnaire (eVQ) cycle ended on Sunday, December 31, 2017. If you did not complete your 2017 eVQ, your ID number(s) were inactivated.

To reactivate your ID Number(s), you must log into the eVQ system and complete the 2017 eVQ. Once you have completed the eVQ and DTSC has received the full payment (if fees are due), your ID Number(s) will automatically be reactivated.

DTSC will not be accepting Form 1358 to reactive numbers that were inactivated because the eVQ was not completed.

The final date to complete the 2017 eVQ will be Sunday, April 01, 2018. After this date, you will no longer be able to log into the eVQ system.

Not required to complete the 2017 eVQ?
If your ID Number(s) were already inactive due to non-completion of a prior year’s eVQ, then you must submit DTSC Form 1358 to reactivate your ID Number(s).

Have a federal EPA ID Number?
To reactivate a federal EPA ID Number, you must complete the 2017 eVQ and submit an EPA Form 8700-12. Federal EPA ID Numbers will not be reactivated without submission of EPA Form 8700-12. A DTSC Form 1358 will not be accepted to reactivate your federal EPA ID Number(s).

PLEASE NOTE: If you are a Transporter with a federal EPA ID Number, you m ust submit a paper “wet” signature EPA Form 8700-12 to DTSC, ATTN: HW Transporter Registration, 1001 I Street, MS 11-A, Sacramento, CA 95814 for any reactivations or updates.

What is the purpose of the Annual EPA ID Number Verification Questionnaire?

Anyone who generates, transports, offers for transport, treats, stores, or disposes of hazardous waste must have a hazardous waste identification (ID) number, which is used to identify the hazardous waste handler and track the hazardous waste from the point of origin to its final disposal ("cradle to grave"). The purpose of this verification is to ensure that the information on record for the EPA ID Number is correct and current.

The annual Verification Questionnaire and fees assessment for hazardous waste ID numbers and hazardous waste manifests is required by Health & Safety Code section 25205.15 and 25205.16. Any generator, transporter, or facility operator who fails to provide information required by the department to verify the accuracy of hazardous waste activity data shall be subject to suspension of any and all identification numbers assigned and to any other enforcement action (Health & Safety Code section 25205.16(c)).
